Welcome to Brookside
Brookside Extended Care is a licensed Intermediate Care Facility (ICF-IID) that supports children and adults with profound developmental and intellectual disabilities. Since opening in 1980, Brookside has played an important role in Ohio’s transition away from institutional settings. Many of our earliest residents came to us from large state-run facilities, including Orient, and several of those individuals remain with us today. Their lives, and the dedication of staff who have supported them over the years, continue to shape the foundation of who we are.
We specialize in supporting individuals with complex medical needs and significant developmental disabilities. Many of our residents are non-verbal, non-ambulatory, and require full assistance with daily living tasks. Some require the use of feeding tubes, tracheostomies, or oxygen, and receive regular nursing interventions like suctioning or seizure monitoring. Other individuals may have common diagnoses’ like cerebral palsy, traumatic and anoxic brain injuries, or conditions related to birth trauma or genetic syndromes.
Our focus is on seeing the person beyond the diagnosis. With twenty-four hour licensed nursing, in-house therapy services, and a deeply committed care team, we provide skilled support that honors each resident’s dignity, preferences, and potential. Brookside is not just a care facility—it’s a home built on trust, structure, and individualized support.

Brookside is one of the few ICFs in Ohio designed to support individuals with profound developmental and medical needs, but what truly sets us apart is the stability and intention behind how we operate. Many of our care team members, especially our nurses and direct support staff, have been with us for twenty years or more. That kind of longevity creates a deep sense of familiarity for our residents and gives families peace of mind knowing their loved one is being supported by people who truly know them. We are proud of the relationships that have formed here, not just between staff and residents, but across families, therapists, and the broader care team.
While Brookside is a licensed care facility, our intention is for it to feel like a home. We build routines that reflect what matters to each person. The environment is structured without feeling clinical. Each space is thoughtfully decorated by families and guardians, with support from case managers when needed, so that every room reflects the individual who lives there.
